If you or your loved one is starting to need some non-medical assistance with their daily life, then we are here to help. We will have some conversations with you to discuss what services you may require and see if we are likely to be a good fit for you. It is our goal to make you or your loved one as independent as possible, as long as possible.
After we have had some conversations, the next step is an elder care evaluation or assessment which we offer at no cost to you. We begin the interview process with an in-depth assessment of lifestyle, medical conditions, and type of assistance needed to maintain independence in the home. You, your family and Advantage meet to jointly determine the scope of care.
During the meeting we fully explain the services available, answer your questions, and guide the important care decisions faced by the family with a loved one in need. Every family meeting is unique; usually, however, the following issues are addressed:
• Matching the appropriate caregiver
• Clarification of daily routines
• Health status and medication circumstances
• Emergency response plan
• Nutritional plans
• Home cleaning needs
• Transportation requirements
• Cost of services
• Other factors relevant to the situation
